Requirements for Admission for the UNDERGRADUATE Department

NEW STUDENTS
1. Report Card (Form 138) or Form 137 original)
2. Birth Certificate (Original and photocopy)
3. Certification of Good Moral Character from the Principal or Guidance Counselor from the previous school
4. Four (4) 2x2 ID photos
5. University Entrance Examination
6. Interview with the Dean of Student Affairs and the Department Dean


TRANSFEREES
Additional requirements:

1. Transfer Credentials
2. Certified true copy of the transcript of records and/or certification of subjects taken from the previous school
3. Evaluation of credits earned
4. 4. Academic and probationary contract with the office of the Student Affairs


FOREIGN STUDENTS
Additional requirements:

1. Evaluation by CHED of the studies abroad
2. Photo copy of:
1. ACR (Alien Certificate of Registration)
2. ICR (Immigration Certificate of Residence)
3. BC (Birth Certificate)
